You should have recieved your 2011 work log books in the mail by now. You should record your 2011 hours worked in the work log book and you should save all of your pay stubs.
THE 2010 YEAR END VACATION CHECKS WERE PUT IN THE MAIL ON WEDNESDAY, FEBRUARY 23, 2011.
INFORMATION PACKAGES: NEW RULES EFFECTIVE SEPTEMBER 1, 2010
The Information Package consists of:
· A cover letter that requests copies of the following documents for the member and each of the member's dependents; Birth Certificate and or passport, Social Security Card, Marriage License, Union Card and Driver License
· Member Information Form
· Family Information Form
· Beneficiary Designation Forms for the Welfare Fund, Vacation Fund, Pension Fund and the Annuity Fund.
By order of the Board of Trustees of the Benefit Funds of Heat and Frost Insulators Local 12, as of September 1, 2010 members who have not complied with the information request that has been sent to them previously, will be sent information requests via certified mail. Each member will have 60 days from the date of the letter to comply with the request. Any member and/or any dependents, who do not comply with the request within 60 days, will be terminated from the Welfare Fund of Heat and Frost Insulators Local 12 benefits. The member and/or member's dependents terminated benefits will not be reinstated until all of the requested information is received by the Benefit Funds Office of Heat and Frost Insulators Local 12. There are no exceptions, either the requested information will be sent into the Benefit Funds Office or your benefits will be terminated. A copy of each of the requested documents must be sent to the Benefit Funds Office. If you have given the Benefit Funds Office your Social Security Card Number or Tax ID Number by phone or by filling out the Member Information Form -YOU STILL HAVE TO SEND IN A COPY OF THE CARD OR YOUR BENEFITS WILL BE TERMINATED.
